Any senior requesting transcripts to be sent to colleges/universities should request them using the form on this page. It is the responsibility of the student to obtain letters of recommendation from teachers if that is required to go with the transcript.  

Note:  When a student applies on-line to a college/university, please read the entire application form because some colleges/universities require a Counselor's Form to accompany the application.  The student must print out the form and give it to the Counseling Department to be completed.

If students have an interest in attending and competing in intercollegiate athletics at an NCAA or NAIA institution they must register with either or both.

7th Semester Grades & Dual Credit Transcript Requests 
Any senior needing their 7th semester grades sent to the colleges that they have applied to before break must request it from the Counseling Office.  No 7th semester grades will be sent to those colleges that were applied to before break unless a student requests it.  Please contact the Counseling Office with any questions.

Transcripts for students who took Dual Credit Courses must be sent directly from Black Hawk College to the College/University that they are applying to.  Colleges/Universities will only accept the transcript directly from Black Hawk.  Call the Black Hawk College Registrar at 796-5345.
